The link below opens a Word Document that contains the form you need to fill out in it's entirety.  When completed and you can submit it three ways.  By saving the Word Document to your PC and filing out the form then print it and mail it using standard mail, or by editing the Word Document saving it to your PC then send it as an attachment using the e-mail address below or phoning the Coldwater Administration Building.  To protect the privacy of students, office staff will verify proof of guardianship or parental rights before you will be issued a ParentConnection account.  By completing this form, you hereby request the creation of ParentConnection account and authorize Coldwater Community Schools to link your child's/children's information to this account.  You will need to provide your drivers license or other photo ID BEFORE you receive the information for your account.

Please fill out the information below.  All the parent/guardian information fields are mandatory.  You must fill out 1 form PER parent/guardian that is requesting access.  You will receive an e-mail confirming your completed application.  After that time you may then come to see Wendy Armstrong at the Administration building, with a photo ID, and pick up your PIN and Account information.
