MHSAA Physical Form (2-Page)

Completed physical forms/cards must be turned in to the Athletic Office in order for student-athletes to be eligible to participate. Blank cards can also be picked up in the Athletic Office at CHS or the Main Offices at CHS and LMS. Latest date for 2021-22 school year eligibility: April 15, 2022.
