Download, print, and fill in these forms before coming to the school district to make your trip that much easier and quicker! There are a few forms that only need to be completed if your son/daughter is attending that building. Here is a quick guide to the forms you might need:

If you currently live within the Coldwater Community School District: 

• Enrollment Packet (Elementary or LMS/CHS)

• Kindergarten Readiness Assessment (if enrolling in Kindergarten)

If you currently live within the Bronson or Quincy school districts:

• Enrollment Packet (Elementary or LMS/CHS)

• Kindergarten Readiness Assessment (if enrolling in Kindergarten)

• Section 105 Form

If you currently live outside the Coldwater, Bronson, or Quincy school districts:

• Enrollment Packet (Elementary or LMS/CHS)

• Kindergarten Readiness Assessment (if enrolling in Kindergarten)

• Section 105c Form

These documents will help assist in the process of transferring your child's records:

• Transcript from student's former school

• Students with an IEP or who need Special Education services: Please bring current documentation

• Free and Reduced Price Meals Household Information Report

• Request for Special Dietary Needs Accommodations
